How to Enroll
Determine if you are a continuing or new student by reading the information below, then follow the directions for returning your form.
What type of student are you?
- Continuing students (those who were enrolled in the spring, 2013 semester) complete a Continuing Student Registration Form, and submit it, with payment, to Continuing Education at the address on the bottom of the form. When registration is complete, your instructor will contact you to set your lesson time.
- New students (those who would like to join the program) complete a New Student Inquiry Form, and submit it to Continuing Education at the address on the bottom of the form. New students are placed with a teacher once continuing students have been placed and space is available. The instructors will contact you to finalize this process. First time students do not pay tuition until they have been placed. Return your completed form using one of the methods described below.

Complete the form, and bring it to the Continuing Education Building at the southwest corner or Main and University in Peoria.
