Typical Worksheets & Forms
Pre-Proposal Form:
The pre-proposal form is completed by a potential client and a sponsoring
faculty member to identify and initiate a potential senior project. The
information included on this form is used to screen projects for course
requirements and in the selection of students with compatible interests. This
information is held confidential and not usually shared with the student team.
Junior Survey Form:
Second semester mechanical engineering juniors complete this form to indicate
their interest and skills. The information included on this form is used to
match students with available projects. This information is held confidential by
the course coordinator.
Client Survey Form:
At the completion of a senior project, all clients are asked to complete a
survey form indicating their level of satisfaction with the project experience
and the results. The information included on this form is used to evaluate the
student performance and as an assessment tool for the course. Feedback from
these forms is used continuously improve the course. This information is also
held confidential and usually shared only with the department faculty.
Check Sheet:
The faculty advisor for a project and the course coordinator complete this sheet
as part of the exit interview for each senior design team. All of the boxes
listed for each team must be checked prior to course completion. Any deficiency
on this form will lead to a grade of "Incomplete" for the students on
the effected team. This information is held confidential and shared only with
the student team.
Student Evaluation Form:
The student teams evaluate each teammate using the evaluation form. These forms
are modeled after those used in industry to evaluate employees. The students
have an opportunity to act as an evaluator and an evaluatee. This information is
held confidential.
Purchase Request Form:
Prior to any purchase, students in the senior design course must complete a
purchase request form and obtain the appropriate signatures. These forms aid in
the strict control of any spending for the course. These forms are modeled after
those used in industry. This information is archived by each student team.
Travel Request Form:
Prior to any travel for course activities, students must complete a travel
request form and obtain the appropriate signatures. These forms identify the
students and the window of travel for students participating in project travel.
