Use the Application for Graduate Admission to apply for admission to the Graduate School and Graduate School funded scholarships. For assistance in filling out the form, applicants should also print the required check-off list below. Please sign and date your application after printing. Your application will NOT be processed without your signature.

CREDIT CARD PAYMENT FORM

Use the Credit Card Payment Form to pay the application fee by credit card. This form must be completed and submitted with the application when paying the application fee using a credit card. The credit card payment form must be signed or the application will not be processed. The credit card payment form may also be used when paying for express mail services and thesis binding fees.
CONFIDENTIAL LETTER OF RECOMMENDATION

The Confidential Letter of Recommendation should be submitted by someone recommending you for admission to the Graduate School at Bradley University.
FINANCIAL INFORMATION AND CERTIFICATION FORM

The Financial Information and Certification Form should only be used by international applicants and their sponsors. The form contains information, instructions and estimated expenses.
TRANSFER AUTHORIZATION FORM FOR F-1 STUDENTS TRANSFERING TO BRADLEY

The Transfer Authorization Form should be used by F-1 students wishing to transfer to Bradley from another US institution.
REQUEST FOR DEFERRAL OF APPLICATION OR ADMISSION

The Request for Deferral Application or Admission Form should be used by applicants who wish to change their application semester, or admitted students who will be unable to begin attending classes during their admission semester.
