If you are planning to complete your requirements for a degree or certificate in May 2007, you must apply for graduation through the Graduate School. The Application Deadline is February 15th. August Graduates
If you are completing your degree requirements in August 2007 but wish to participat in the May Commencement Ceremony please have your Application for Graduation returned to the Graduate School by February 15th.

REMOVAL OF CONDITIONAL STATUS
Completed by those who have met condition of admission. If you were admitted on conditional status, the terms of your condition were listed in your admission letter.

CHANGE OF ADDRESS AND/OR NAME
Complete this form when you have a permanent local address or you have changed addresses or your name. Remember: You must also change your information with the Registrar's Office! You can do this on line by using Webster. Submitting the Change of Address and/or Name form will only change your information in the Graduate School, it will not change your information in the Registrar's Office.
