Computing Login Request Form

The Outlook Request for Staff Logins form will be used for documenting access to administrative systems. Administrative systems are defined as all computer systems other than Email, and academic systems.
You need to run Outlook mail client to use this form. If you are not running the Outlook client, send an email to logins@bumail.bradley.edu. Provide the individual's name, social security number and the access required.
Creation of Computer Accounts
To access this form, from Outlook choose Tools->Forms->Choose Form
then select Staff login request.
This will bring up the login request form. The form is rather large so I suggest you maximize the form so you can see the whole form on your screen.
When it starts there are 2 tabs at the top - one for Identification and another for accounts.
Identification
Based on what you check for Employee status, you may need to supply additional information. New staff will require a SSN number and a start date. Students will require a student ID number.
Accounts
The accounts tab has 4 check boxes for what basic access you desire. The first box is Exchange - email. Borus and Regi are NetWare servers, so you need to check that box for office access to either of those servers. Alex is an AD Security Group. When you check either the NetWare and/or the AD Security Group boxes, a new tab is displayed "Additional Info".
Additional Info
The additional info is where you tell us the specific type of access the person needs. There is a check box for "Standard Office Permissions" - if you have such a thing. Otherwise you will need to type in a description of the type of permissions needed.
