Format and Requirements

1. Submission Time and Date: on or before 3:00 p.m. on Monday, November 2, 2009. BR 380.
2. Style: Personal Essay, a type of creative nonfiction, ranging from autobiography to first person commentary.
3. Length: Not to exceed 3000 of your own words, excluding quotations from other sources.
4. Format: Essay must be typed double-space, pages numbered. Title of the essay must appear
   on top of the first page. Your name must appear ONLY on this form.
5. Completed, signed form required.
