6.02.02 Compromised Account Notification
I. Purpose
This procedure is used by Bradley University computer and network user to notify the university that their account may have been compromised and the university will secure the account.

II. Description
The account holder can notify Computing Services of a potential account problem in two ways:
- In person at Computing Services in Morgan 205. A government-issued picture ID is required to verify identity. Acceptable IDs include state-issued driver license, state-issued ID card, or US government-issued student Visa. The account is secured and an account validation letter is printed and handed to the individual.
- In person or by phone to the Service Desk.
- For phone reporting, the account holder will need to verify identity by providing the last four digits of their social security number. If an individual does not have a social security number on file, the phone option is not available. The account is secured and an account validation letter is printed the next business day. That letter is mailed to the last known address of the individual.
- In person, a government-issued picture ID is required to verify identity. Acceptable IDs include state-issued driver license, state-issued ID card, or US government-issued student Visa. The account is secured and an account validation letter is printed. That letter may be picked up in Morgan 205.
In either case the individual should provide their BUnetID and a date they suspect the account was compromised. That data may be necessary to provide a “Last known valid address” for a student.

This procedure applies to all Bradley University computer and network users.
